Key Clinical Takeaways

  • What It Is: A 30-minute in-office treatment that creates controlled micro-channels in the scalp to stimulate the follicle and help prescriptive minoxidil and finasteride serum absorb more effectively.
  • Who It Is For: People with thinning hair or a receding hairline from androgenetic (pattern) hair loss who want to protect and strengthen the hair they have.
  • The Donaldson Difference: A credentialed, provider-led hair restoration plan built around this as a single focused approach rather than offered as part of a transplant.

Science Behind The Solution

Support What You Have, Strengthen What’s Thinning

Finasteride blocks 5-alpha-reductase. This is the enzyme that converts testosterone into DHT, which is the hormone that drives follicle miniaturization in androgenetic hair loss. This means we address the underlying cause of thinning rather than only stimulating growth. Minoxidil is converted by follicle enzymes into minoxidil sulfate, which increases blood flow and oxygen to the follicle. This prolongs the growth (anagen) phase while shortening the resting (telogen) phase.
Microneedling — kept to a depth of no more than 1mm — creates controlled micro-channels that trigger the scalp’s own healing and growth response and help the topical serum penetrate more effectively, so the three mechanisms work in complementary ways.

A Simple Car Metaphor to Drive Home the Process

Finasteride acts as the brakes on hormone-driven thinning. Minoxidil acts as the gas pedal for growth. Microneedling puts an expert driver in the driver’s seat, and Nutrafol supplements act as premium fuel. Ariessence PDGF+ is the optional upgrade that can take performance to the next level.

What Happens During a Scalp Microneedling Treatment

  • 01

    Consultation & Medication Order

Your provider reviews your hair loss pattern, health history and goals; your minoxidil 5% and finasteride 0.1% serum is compounded specifically for you before your first session.

  • 02

    Treatment Preparation

Your provider prepares your scalp for treatment. We do not use numbing, because it can work against the blood flow response we want at the follicle. If you would like it, Pro-Nox is available to help you manage discomfort.

  • 03

    Microneedling Begins

Your provider treats the scalp with controlled microneedling no deeper than 1mm, and the session takes about 30 minutes.

  • 04

    Your At-Home Plan

You leave our office with your serum, your personal CIT roller and clear written post-treatment instructions for the days ahead.

  • 05

    Your Next Treatment

Most patients complete three sessions, spaced four weeks apart. These are most often planned and booked ahead at your consultation.

What Happens After Each Session

  • First 24 Hours: Apply nothing to the scalp. That means no serum, no roller and no products of any kind, so the skin can begin healing undisturbed.
  • After 24 Hours — Ongoing: Apply your minoxidil and finasteride serum twice daily and use your CIT roller at least once per week as part of your routine.

Frequently Asked Questions About Scalp Microneedling for Hair Restoration

Women and men with thinning hair or a receding hairline from pattern hair loss who still have hair to retain and strengthen are typically the best candidates. Earlier treatment generally gives the medication and your follicles more to work with.

Your provider reviews your hair loss pattern, health history and current medications at your consultation to confirm the treatment is appropriate for you. Finasteride also carries pregnancy-related precautions, so your provider will also discuss this with you during the consultation.

Mild redness, tenderness or flaking can occur on the scalp and is typically temporary. Temporary shedding is also normal as thinner hairs make room for stronger ones, so we encourage you to stay consistent with your serum. Recovery involves no intensive downtime, but you should follow the instructions given to you by your provider after your session.

  • Applying anything to your scalp — no serum, no roller and no products of any kind — for the first 24 hours
  • Wearing a hat for 8 – 24 hours
  • Washing your hair for 24 hours
  • Heavy sweating and exercise for 24 – 48 hours to mitigate infection risk
  • Direct sunlight for 48 – 72 hours; avoid prolonged sun exposure for up to a week
  • NSAIDs (ibuprofen, aspirin, etc.) for up to a week

Note: Continue your Nutrafol routine as usual. After 24 hours, you can begin applying your serum.

Comfort & Healing Tips

  • After 24 hours, apply your minoxidil and finasteride serum twice daily.
  • Use your CIT roller at least once per week as part of your routine.
  • Acetaminophen can be used for comfort management, as this medication does not suppress the body’s natural inflammatory response, which is vital for a strong outcome.
  • Do Not Scratch: Mild itchiness during healing is completely normal.

Expect temporary shedding first, then gradual regrowth, with visible changes typically appearing around three to six months. Early shedding is completely normal. Those hairs are what you likely would have lost over the next six months, and as the follicle gets bigger, it pushes the thin hair out of the way.

Results build gradually after the initial shedding phase, so consistent serum use between sessions matters as much as the sessions themselves.

Apply nothing to your scalp for the first 24 hours. This means no serum, no roller and no products of any kind. After 24 hours, apply your minoxidil and finasteride serum twice daily and use your CIT roller at least once per week as part of your regular routine.
Your provider reviews this timeline with you before you leave your first appointment.

Most patients need three scalp microneedling treatments, spaced four weeks apart. Three is our practice minimum, and the total can vary from patient to patient based on how your hair responds. Your serum and roller routine continues between and after sessions, which helps maintain your progress.

Scalp microneedling is not exactly comfortable, but it is typically well tolerated by our patients. We do not numb the scalp because numbing can work against the blood flow response we are trying to stimulate at the follicle.
What we do offer is Pro-Nox, a safe, self-administered, fast-acting nitrous oxide option that helps you manage discomfort in the moment. The effects of Pro-Nox are short-lasting, so you’re able to drive home after your session.
Most likely, yes, if you want to maintain your results for as long as possible. Minoxidil and finasteride work only while you use them, so stopping typically allows the underlying hair loss process to resume.
We understand this is a real commitment. That is why we build your routine around a simple twice-daily serum that fits into your day. Many patients also add Nutrafol supplements to further support healthy hair growth.
Research suggests scalp microneedling works best as part of a combined plan rather than on its own. A 2024 meta-analysis of eight randomized trials found that microneedling plus 5% minoxidil increased hair count more than minoxidil alone, while microneedling by itself showed limited effect.
Results vary from patient to patient, and the research is still evolving, so we set realistic expectations at your consultation.
No, we do not perform hair transplant surgery. We specialize in nonsurgical hair restoration, including scalp microneedling and PRP, for people who want to retain and strengthen the hair they have.
If a transplant is the right fit for your goals, we are happy to refer you to one of the trusted local surgeons we work with.
