The Most Effective Treatment For You Depends on Skin Laxity & Elasticity

Whether a treatment will be effective or not varies from person-to-person, depending on two things: the level of skin laxity (mild, moderate, or severe) and the overall elasticity of the skin. For example, an individual in their 20s or 30s with mild or moderate loose skin will have relatively higher skin elasticity and, therefore, will be more likely to see loose skin improve over time with regular non-invasive treatments.

Moderate to severe skin laxity is more common among individuals in their 50s or 60s. This population is more likely to have relatively lower skin elasticity, so their skin may be less responsive to non-surgical treatments. In this case, surgical skin tightening may be a more effective path to correct your concerns.

Ultherapy

Non-invasive skin tightening treatments like Ultherapy are more targeted and go deeper to address the underlying causes of mild to moderate loose skin. Ultherapy is a non-surgical skin tightening and lifting treatment that uses ultrasonic energy to send waves deep within the skin that stimulate collagen production. Because it works on a deeper level than other treatments, Ultherapy is a great preventative treatment for individuals seeing the beginnings of aging skin, rather than a fix to severe sagging or wrinkling. It is primarily intended to lift and tighten the skin of the brow, chin, jowls, neck, and upper chest, but can also be used to target other specific areas of concern.

Halo

Halo laser treatment

Halo Laser Skin Resurfacing is a hybrid fractional laser treatment that addresses a number of visible skin concerns and offers skin tightening benefits from inducing collagen production in the healing process. For patients with mild to moderate skin laxity, Halo can result in more youthful, tighter-looking skin. Although the Halo laser is not primarily for skin tightening, patients usually see and feel a skin tightening effect. Minimally-Invasive Skin Tightening Treatments

Renuvion (J Plasma) Skin Tightening

Renuvion (also known as J Plasma) is a minimally-invasive procedure that uses a combination of radiofrequency energy and helium gas to virtually tighten the skin of the face, neck, and body. A wand emitting the stream of energy is gently passed underneath (in some cases overtop) the dermal layer to cause contraction of the skin. Injectables

Botox and fillers are not “skin tightening treatments”, but they do offer benefits that address signs of mild to moderate skin laxity, which is why we include them here.

Aging skin can cause wrinkles, folds, and droopiness to form, which can be corrected using minimally-invasive injectables like Botox and dermal fillers for the face. Botox works by softening the appearance fine lines and wrinkles in the skin by relaxing the muscle underneath that causes the crease to form. Fillers restore volume under the skin to an area where volume has been lost and skin has begun to droop or fold as a result (ex. the jawline, or nasolabial folds and marionette lines). Neither injectable treatment is used to reverse skin laxity, but they can help to improve common concerns associated with it.

Surgical Skin Tightening Procedures

To successfully treat more significant loose skin, surgery is usually the best option.

With surgical skin tightening procedures, excess skin is removed surgically and the remaining skin is re-draped tighter than before. This option is the gold-standard is for patients who have moderate to severe skin laxity as a result of aging or weight loss but can be ideal for mild to moderate skin laxity as well.

Here are some of our most effective surgical solutions performed by Dr. Donaldson & Dr. Sieffert.

Tummy Tuck

A tummy tuck is a procedure in which excess skin and fat are surgically removed from the abdomen (and the underlying muscles tightened) to improve its contours. Stretched skin in this area will not always retract on its own, making surgery the only option to return the abdomen to its original form.

Anyone who has undergone pregnancy or weight loss, or who just simply struggles with excess skin of the belly, can benefit from this kind of surgical tightening. Scarring for a tummy tuck is minimal and easily concealed with undergarments or bathing suits, and our tummy tuck patients are thrilled with their results. Their only regret is they wished they had done is sooner!

Body Lift

A body lift, or circumferential body contouring, is a procedure in which excess skin of the entire abdominal front, back, and sides is removed for a tighter trunk. Usually performed on individuals with severe skin laxity of the body, this procedure is common amongst individuals who have undergone significant weight loss and are left with excess skin as a result. A body lift can also encompass surgical tightening of multiple areas including the arms, chest, upper and lower back, buttocks, and thighs — the surgery is always customized to address the patient’s concerns.

Arm and Thigh Lift

Arm lifts and thigh lifts are procedures in which sagging excess skin of the upper arms and thighs is removed, often solving cosmetic concerns of the colloquial “bat wings” and sagging skin of the inner thighs and above the knee. They can be performed individually or together, depending on a patient’s wishes. These two areas are very likely to loosen with age, so the majority of arm and thigh lift candidates simply wish to see their extremities restored to a more youthful-looking state. Scarring is likely to be visible in these areas, so please discuss your concerns with your surgeon prior to surgery.

Face and Neck Lift

Facelifts and neck lifts are also common amongst individuals with concerns of drooping skin. As exterior representations of how we present ourselves to the world, the areas of the face and neck are often ones of prime importance — but, unfortunately, they can be some of the first areas to show the effects of aging. In a facelift and/or neck lift, excess skin is excised by your surgeon and the skin is laid tauter than it was before. Specific areas like the lower or upper face can be targeted, as in the case of a brow lift. Scarring is very discrete and easily concealed.

Eyelid Lift

Upper and lower blepharoplasty, or eyelid surgery, are surgical procedures in which excess skin of the upper and lower lid is carefully removed, and the remaining skin joined back together for younger, fresher-looking eyes. If you have what some might describe as overly-hooded eyes, or suffer from large bags underneath, you are likely a good candidate for an eyelid lift. Sometimes fat pads in the lower eyelid may also be removed to reduce puffiness. The procedure is straightforward and scarring is virtually invisible.

The COVID Vaccine & Fillers: The Best Way to Do Both Safely

As Ohio vaccinations increase, questions have arisen about possible contra-indications for the COVID vaccine. Many injectable patients have been left wondering: “should I wait to get the COVID-19 vaccine if I have recently had Botox or fillers?”, and “should I wait to receive Botox and fillers if I am newly vaccinated?”

As leading plastic surgeons in Columbus, Ohio, we use the most recent data available to provide guidance on the safety surrounding Botox, fillers, & the COVID vaccine, and how long is an appropriate amount of time to wait in between them. In order to decrease your potential for late-onset swelling, we recommend you err on the side of caution by scheduling time between your injections and the vaccine.

Receiving Injectables After the Vaccine

It is safe to receive treatments like Botox and fillers after receiving the COVID vaccine. The vaccine is not a contra-indication for Botox or fillers, meaning that if you are already vaccinated (or are planning to become vaccinated before your injectable visit), you have no reason to worry!

However, our injectors do recommend waiting approximately 2 weeks after your vaccine to come in for a Botox/filler treatment. The reason for this is to wait for any potential symptoms from the vaccine subside; not everyone experiences noticeable symptoms after vaccination, but your body will typically have some sort of an inflammatory response to the vaccine (which is what you want in order to create the antibodies to the virus) that you should wait out before resuming or trying new cosmetic treatments.


Receiving the Vaccine After Injectables

Is it safe to get the vaccine after Botox?

Yes! In our experience, we have not observed any instance where a patient has experienced side effects related to their Botox treatment after receiving the COVID vaccine, nor from other wrinkle relaxers like Dysport, Xeomin, or Jeuveau.

Is it safe to get the vaccine after fillers?

Yes. It is safe to get the vaccine if you have a history of fillers. However, there is a very slight possibility you may receive some swelling afterward. There have been three cases reported by the FDA (out of a sample of 15,184) in which patients have experienced late-onset swelling in the area of their filler following the COVID-19 Moderna vaccination, most likely related to the body’s inflammatory response.

Reported symptoms of these cases were mild and quickly resolved. It is normal to be swollen for the couple of weeks after fillers. However, if swelling returns three to four weeks after fillers, this may be a symptom of your body’s immunological response following the vaccine.

In fact, this type of delayed-onset swelling isn’t uncommon. It is fairly normal to see residual swelling of the injection area anytime there is a diffuse inflammation response occurring in the body. Inflammation can be caused by any number of things that trigger an immune reaction — certain foods, environmental factors, and, of course, viruses can all cause varied levels of inflammation.

Guidance provided by the American Society of Plastic Surgeons (ASPS) states that, for these reasons, they do not discourage vaccination for any individual with a history of filler or Botox use.


Our Recommendations

Plan out your appointments. We agree with the ASPS that the risk of swelling is so minimal that it should not dissuade you from receiving the vaccine, nor cosmetic injectables. The benefits of receiving the vaccine vastly outweigh any potential risks or side effects like those described in the FDA’s report. It should not matter the order in which you receive them, as long as you provide yourself an appropriate buffer period to allow your body’s natural immune response to settle.

How Long Should You Wait After Your Filler to Get the Vaccine?

Give yourself a minimum of two-weeks between your appointments, just to be safe. Make an informed decision regarding your vaccination, and be sure to ask your injector any questions you have before your treatment. As healthcare providers, we encourage vaccination against COVID-19. We will continue to follow the CDC’s guidelines as they apply to healthcare facilities, and closely monitor the FDA’s literature as it relates to dermal fillers and the COVID vaccine.

7 Useful Tips To Give You More Youthful-Looking Lips

We are proud to say that no one appreciates the importance of lips more than the highly-trained aesthetic providers at Donaldson Plastic Surgery. We can think of no better way to honor this day by sharing our top tips for youthful-looking lips.

1. Lip filler

Lip filler is one of our most popular treatments, with thousands of patients having had received their lip filler treatments from our office. Volume in lips is a hallmark sign of youthfulness — as we age, we lose this volume over time. Lip filler is made of hyaluronic acid (HA), which is a protein compound found naturally in our bodies. This substance is an excellent building-block for restoring lost volume in the lips, and can even allow us to create youthful-looking lips that will compliment your natural features. We tend to be give you a more conservative, natural treatment, especially if it is your first treatment. We can always add more filler with a touch-up!

2. Botox “Lip-flip”

Have you heard of a lip-flip? This lesser-known injectable technique has recently become a desirable alternative for those who are not interested in traditional lip fillers. Rather than adding volume, the Botox lip-flip instead relaxes the orbicularis oris muscle, allowing the top lip to relax and unfurl. “Flipping” the top lip slightly outwards this way results in a more noticeable projection of the cupid’s bow and upper tubercle (the small, rounded middle part of the upper lip), which gives the lips a fuller-looking appearance. Results can vary with the lip-flip, but it often is a good way to try out minimally-invasive techniques for more youthful-looking lips.

7. ZO Exfoliating Polish

Exfoliation is an important part of maintaining youthful-looking lips! A fan-favorite from renowned medical-grade skincare line ZO Skin Health, the Exfoliating Polish uses microdermabrasion crystals to finely and gently remove excess dead skin cells from the skin’s surface layer. It’s back-to-school time at my house, so I thought I should take the opportunity to share with our patients about one of our most popular nonsurgical treatments Welcome to Filler 101!

I am a certified Physician Assistant of 12 years, specializing in plastic surgery and non-surgical aesthetic treatments. I am very experienced and well-versed in the topic of facial fillers and nonsurgical aesthetic treatments, and I can’t wait to dive in with you! Here are some of the most important facts about fillers that you should know.


What does filler do?

Dermal filler is a soft, malleable substance made of hyaluronic acid that is injected underneath the skin. If you ask a highly-trained injector, they will tell you that the purpose of filler injections is to restore lost volume due to aging & to bring balance to a person’s face by controlling its proportions. Either by enhancing or de-enhancing certain facial features, filler is injected to create a smoother, more attractive appearance.

Proportions: Where Science Meets Beauty

Filler uses the science of proportion in order to make certain features appear more or less prominent. Your injector should therefore have a advanced understanding of facial anatomy, product knowledge, and aesthetic training for you to achieve a technically beautiful result. Subtle manipulations and micro-adjustments (with 1ml of filler or less) can make a world of difference to a patient’s appearance, and to their own self-image.

That being said, and facial sculpting and rejuvenation takes a fair amount of artistry and creativity on the behalf of your injector. Not every face requires a straightforward approach, so your injector’s style definitely comes into play!


What are the different kinds of filler?

Filler comes in a lot of different forms, and they are formulated for different purposes and for use in different treatment areas. We primarily carry the Juvederm collection of fillers, which contain the following three favorites of mine:

Volbella

Volbella is an injectable hyaluronic acid filler that can be considered a ‘cousin’ to Juvederm.  It is used to create subtle volume at lips and can also treat fine lines and wrinkles around the mouth.  Because of the way Volbella is manufactured, results can last up to 1 year post-treatment.  It is known to create a ‘softer’ look when compared to Juvederm.  Volbella is a fantastic lip filler for first time patients!

Voluma

Voluma is the hyaluronic acid filler of choice to treat age-related volume loss in the face.  It is the most viscous (thickest/heaviest) of the fillers and it tends to last the longest – up to 2 years post-treatment.  Voluma is not only injected below the skin in the subcutaneous level, it can be strategically placed along the cheek bones to recreate a ‘strut’ for the aging midface, restoring structure and contour. 

Vollure

Vollure is the middle child of the hyaluronic acid fillers – it is less firm than Voluma but holds more structure than Volbella.  Vollure is the optimal filler to use most commonly in the lower facial moderate to severe wrinkles, including nasolabial folds.  It is also versatile enough to correct deep tear trough deformities.  Results from these injections typically last around 18 months. Other fillers that we offer
