Neurotoxin injections (such as Botox, Xeomin, Dysport, Jeuveau, etc.) are used to treat wrinkles caused by muscle activity—most commonly in the forehead and around the eyes. They can also be used for prevention if you are only just starting to see wrinkles surface! The injections are relatively painless, only take a few moments to complete, and only need to be repeated roughly every 3-4 months (so you may be starting to see those fine lines and wrinkles return while on quarantine). My single rule for neurotoxin injections is simple: no exercise for 24 hours afterwards.

Filler is a substance made of hyaluronic acid (HA), which is a naturally occurring substance in your skin. It is used to plump areas of the face and body that have experienced volume loss, or to shape areas of the face like the nose, chin, and jawline. Different fillers (Ultra and Ultra Plus, Voluma, Vollure, Volbella, Restylane, Radiesse, etc.) are designed with different measures of thickness and longevity; they can last anywhere from 1-2 years depending on the product used and the location injected. Some of my favorite locations for filler are the lips, cheeks, jawline, temples, and nasolabial folds. The treatment generally lasts 30 minutes (most of that time is spent discussing your needs and desires), and you can expect mild, but not unbearable, discomfort.
I suggest planning filler injections at least a week ahead of any major event, as swelling and bruising is a possibility with any filler injection. As mentioned above, we offer complimentary make-up touch ups after your injectable visit, so you can leave our office looking and feeling like you!
Perhaps you’ve been experimenting with making your own facials at home to keep your skin healthy and glowing, but need a little something extra to brighten or moisturize as you’re getting back to work. Everyone has that “one area” that, no matter how hard you exercise and diet, never seems to go away; this is where CoolSculpting is of benefit! The advanced CoolSculpting technology freezes localized areas of fat, causing damage and, ultimately, death, of the fat cell itself. Over the ensuing 2 to 3 months, the body removes the damaged fat cells from the area, causing a reduction of up to 25% of the fat present in the treatment area.
Each CoolSculpting treatment, or “cycle”, lasts 1 hour. You can expect mild discomfort and tingling as the treatment begins and ends (as the skin cools and re-warms), and an area of numbness that returns to normal between 2 days and 3 weeks afterwards. As with any minimally invasive body contouring technology, multiple treatments in the same area are required to get a great result.
CoolTone is the newest non-invasive body contouring tool at our practice, and one that I am personally pretty excited about! It uses electromagnetic stimulation to give your abdominal, gluteal, or thigh muscles (other treatment areas coming soon) a intense 30-minute workout, with the result being visible toning and strengthening. This is a perfect solution for anyone who wants firmer abs, tighter glutes, or more defined thighs!
CoolTone treatments begin with a series of four 30-minute “workouts” performed over a 2 week period, with subsequent maintenance treatments performed on a quarterly to monthly basis. Your physician is a great resource for questions about the procedure, but it never hurts to do a little outside research. That way you won’t forget to ask any questions when meeting with your doctor. When it comes to choosing a provider, look for someone who is board-certified and has a well-trained staff.
What’s inside the body can affect your reaction to the treatment. You might need to alter your diet a bit in the two weeks leading up to your appointment. Avoid aspirin, non-steroidal anti-inflammatory medications — such as Motrin and Advil — Vitamin E, and fish oil. All of these can increase bruising following the treatment. Eating more leafy greens and Vitamin K also can help prevent bruising/swelling.
One of the most important things you can do is build in recovery time. There may be visible redness and swelling immediately following your appointment. For added discretion, book injections on a Friday or take a day off from work following the injection.

We’ve laid out an easy to follow guide for scheduling all of your prenuptial procedures. Before we get started, a quick note from Dr. Donaldson: Starting your aesthetic treatments 12 months out is by no means imperative for a beautiful wedding. Whether it is a year away or the week before ‘I do’, there are treatment options available to help you look and feel like the very best version of you.
If you are considering a procedure like rhinoplasty, breast augmentation, or liposuction before the big day, we recommend scheduling them a year out. This will give you a generous amount of time to heal from the surgery so that you won’t need to be focused on your recovery in the months leading up. If your procedure affects your figure, this also gives you time to consider how your wedding attire may fit differently before and after.
Laser hair removal is a very common treatment leading up to the big day, and for good reason; who should worry about under arm stubble when tossing the bouquet, or the bikini line on their honeymoon? Laser treatments are typically done in a series with an average of six sessions to maximize results. With four to six weeks between each treatment, eight months should provide the right amount of time to smooth those areas.
If your wedding is at least six months away, Ultherapy is an excellent alternative to give you the benefits of a face lift with no downtime. If surgery is out of the question due to time, finances, or personal preference, another option to consider is CoolSculpting. It is a non-invasive treatment that has become increasingly popular for treating “problem areas” that don’t resolve with pre-wedding diet and exercise. CoolSculpting is often performed on the arms, back, inner and outer thighs, abdomen, love handles, and under the chin.

COLUMBUS, Ohio—The same person who fills your cavities also could be filling your wrinkles—with Botox.
Some dentists now are offering cosmetic procedures such as Botox.
Dr. Michael Firouzian started offering Botox and other dermal fillers after some of his patients asked about it.
“I found out that it’s something within our scope that we can do. I went down to Florida and did some training, and I had a plastic surgeon come in for a whole week and I trained with him,“ Firouzian said.
Plastic Surgeon Dr. Jeffrey Donaldson said that a week of training can’t compare to years of training required for board certification in plastic surgery.
He said patients should stick with plastic surgeons for any cosmetic procedure.
“From my end, a plastic surgeon has specific training in cosmetic services and reconstructive services. It’s the safest bet to have those procedures done,“ Donaldson said. “Materials like Botox injected into the wrong anatomical area can cause things like eyelids to drop and different facial expressions to disappear.“
Firouzian said he knows and understands the risks.
“As dentists, we are fully trained in knowing all the anatomy and knowing where all the nerves are. This is a service we offer to our own patients with a full understanding that there are certain guidelines we have to abide by,“ Firouzian said.
The Ohio State Dental Board has ruled that treatments, such as Botox, are within the scope of practice for dentists as long as they are limited to the facial areas.
The American Society of Plastic Surgeons said Botox falls squarely within the practice of medicine and should only be done by a board-certified plastic surgeon.

Several weeks ago, the company Allergan launched a new and improved version of Juvéderm that contains lidocaine — a numbing medication similar to novocaine. Their studies have shown that 9 out of 10 patients are more comfortable with the lidocaine than without, and so far Dr. Donaldson’s patients agree. Procedures that might have otherwise required a nerve block (direct numbing of a facial nerve) can now be done safely and comfortably with the new filler alone.
In the last several days a competitor, Medicis, announced the availability of their latest products: Restylane-L and Perlane-L. These next-generation fillers also contain lidocaine for a more pleasant cosmetic experience. In the past decade, a new generation of plastic surgeons has turned upside down the traditional thinking about aging, beauty and the aesthetic ideal. It used to be assumed that aging effects were due to gravity and stretching, and that tissues sagged and fell over time. While this may be true to a certain extent, there is another fundamental process at work: loss of volume.
Technological breakthroughs in 3-D imaging, CAT scanning and MRI’s clearly demonstrate a decrease in the amount of bone, size of fat pads, quantity of muscle fiber and thickness of skin over time. In contrast, high-definition photographs show that skin landmarks such as moles and freckles do not descend appreciably as a person ages. These studies suggest that the wrinkling and hollowness that characterize old age are similar to a grape becoming a raisin, or a helium balloon that has deflated — it is due to volume loss from deeper within the body, and not just sagging of the skin itself.
This new understanding has major implications for the treatment and prevention of aging. The face lift provides an excellent example. The old mantra was to lift, tighten, stretch and smooth the skin, but now we know that the skin is merely redraping over less underlying tissue. If only the skin is tightened, the effect may appear as if a patient is standing in a wind tunnel — the skin is swept back smooth and tight, but overlying a hollowed and sunken framework. The more pure anatomic approach is to rejuvenate the face by restoring inner volume where it has faded. This may include the addition of fat grafts,duplication of underlying fascia and muscular tissue, placement of implants, and/or injection of hyaluronic acid filler such as Restylane and Juvéderm in combination with a face lift. When the deeper tissues are filled, then the skin is pushed out and wrinkles smooth more naturally.

(1) How would you define a fat transfer plastic surgery procedure to readers who don’t know what it is?
Fat transfer is a minimally-invasive surgical procedure during which a person’s own fat is harvested though needles, or cannulas, then purified outside the body and re-injected into areas that need more volume. In essence, fat is taken from wherever it is unwanted and transferred to wherever it is wanted.
(2) Are these types of procedures popular?
Very. Just in the last ten years, many advances have been made in the harvesting, purification and injection techniques. Many plastic surgical training programs have begun to incorporate fat transfer into their curricula, and it is routinely discussed at national and international plastic surgical
(3) What are the different kinds of fat transfer procedures? How are they performed?
Fat transfer procedures may be categorized as cosmetic procedures and reconstructive procedures. Cosmetic purposes primarily include facial rejuvenation (cheek augmentation, lip augmentation, lower eyelid treatment, nasal contouring, wrinkle/crease filling, acne scar treatment) and buttock augmentation; reconstructive purposes include filling of post-cancer defects, treatment of radiation damage and scar revision to name a few.
They are all performed using sterile technique in an operating room, and they all include various methods for (1) fat harvesting that is similar to liposuction, (2) fat purification using centrifuges, strainers and/or wicks, and (3) fat injection using syringes and specialized needles or cannulas. The fat must then be painstakingly placed in tiny particles throughout the surrounding tissues in a latticework, or honeycomb pattern, so that they take on blood supply and become part of the body once again.
(4) What types of fat transfer procedures to you perform in your practice?
Facial rejuvenation, buttock augmentation, filling of breast cancer defects and scar revision.
(5) What are the pros and cons of these different fat transfer surgeries?
Pros: Permanent, natural, safe, minimally-invasive, affordable. Cons: Variable results depending upon surgeon and patient.
(6) What are the side effects of fat transfer surgeries?
One side effect is favorable: stem cell activity. Fat is rich in stem cells, and current research is attempting to understand how fat transfer procedures may maximize their utility. Many patients describe improvements in skin tone, clarity, texture and pore density above areas of fat transfer, which may possibly be due to stem cell differentiation into skin cells. Negative side effects can include oil cysts, fat necrosis or granulomas.
(7) What should a patient consider before considering this type of plastic surgery? How are these considerations different from other plastic surgery procedures?
First, the results may be unpredictable. They are highly dependent upon the surgical technique and the surgeon’s experience, but also the patient’s own fat and blood supply. Second, the results change significantly over time. Most studies show that 50-70% of injected fat cells survive, which means that up to half of the immediate result may be lost. This leads most surgeons to “over-correct” during the procedure, knowing that the final outcome will be less. Finally, it is a permanent solution, unlike fillers such as Restylane or Juvéderm that fade away after a year. Some patients use temporary fillers to test their appearance before committing to fat transfer.
(8) What do you see as the future of fat transfer plastic surgery? Where is it headed?
One of the most exciting and controversial applications for fat transfer is in cosmetic breast augmentation. Many women (and surgeons) are thrilled by the idea of taking fat from the belly, or the hips and using it to enlarge the breasts. These techniques are being developed and refined, but many questions about long-term safety and the effect on breast cancer screening will need to be answered before this becomes mainstream. Also, more breakthroughs in the delivery of stem cells through fat transfer will likely be seen soon.

Fat transfer may be done under local or general anesthesia, depending upon the extent of the procedure and the areas of treatment. Minor facial rejuvenation typically costs around $2000, major facial rejuvenation costs around $5000, and buttock augmentation costs around $10,000.
