When it comes to cosmetic surgery, <a href="http://www.donaldsonplasticsurgery.com/breast-augmentation-columbus-ohio/">breast augmentation</a> is the number one procedure done in the US.</p>
<p>There were 289,000 done last year alone.</p>
<p>Doctors are always looking for better ways to perform the procedure &#8212; ways that give patients better results with less down time.</p>
<p>Plastic and reconstructive surgeon Dr. Jeffrey Donaldson, of Columbus, spoke about what he&#8217;s doing.</p>
<p>&#8220;We&#8217;re doing endoscopic, transaxilliary <a href="http://www.donaldsonplasticsurgery.com/breast-augmentation-columbus-ohio/">breast augmentation</a>.  It&#8217;s positioning the <a href="http://www.donaldsonplasticsurgery.com/breast-augmentation-columbus-ohio/">breast implants </a>through the armpits and under the muscle,&#8221; Dr. Donaldson explained.</p>
<p>Typically surgeons cut just under or on the breast, leaving a scar, but Dr. Donaldson&#8217;s procedure creates only a tiny scar in the armpit, leaving no visible scar on the breast.</p>
<p>&#8220;These scars are really well hidden, way up high in the armpits, so this surgery is appealing because it leaves the scars off the breasts,&#8221; said Donaldson.</p>
<p>First a small incision, about a half-inch wide is made.</p>
<p>Then space is created for the implant, and then he deflates the implant.  The doctor then fills the implant with saline once inside.</p>
<p>The procedure costs about $5,000 and isn&#8217;t for everyone.</p>
<p>Dr. Donaldson said the ideal patient is young and looking for a natural look.</p>
<p>Patients will experience a week of swelling and bruising.</p>

			<content:encoded><![CDATA[<p>Filling, roundness, softness, plumping are all desired goals within the latest volumetric philosophy that has permeated plastic surgical literature and discussions.</p>
<p>In the past decade, a new generation of plastic surgeons has turned upside down the traditional thinking about aging, beauty and the aesthetic ideal.  It used to be assumed that aging effects were due to gravity and stretching, and that tissues sagged and fell over time. While this may be true to a certain extent, there is another fundamental process at work: loss of volume.</p>
<p>Technological breakthroughs in 3-D imaging, CAT scanning and MRI&#8217;s clearly demonstrate a decrease in the amount of bone, size of fat pads, quantity of muscle fiber and thickness of skin over time.  In contrast, high-definition photographs show that skin landmarks such as moles and freckles do not descend appreciably as a person ages.  These studies suggest that the wrinkling and hollowness that characterize old age are similar to a grape becoming a raisin, or a helium balloon that has deflated &#8212; it is due to volume loss from deeper within the body, and not just sagging of the skin itself.  <img class="alignnone" title="Juvederm" src="http://farm3.static.flickr.com/2796/4387672411_0c06d73289.jpg" alt="" width="203" height="152" /></p>
<p>This new understanding has major implications for the treatment and prevention of aging.  The <a href="http://www.donaldsonplasticsurgery.com/face-lift-surgery-columbus-ohio/">face lift </a>provides an excellent example.  The old mantra was to lift, tighten, stretch and smooth the skin, but now we know that the skin is merely redraping over less underlying tissue. If only the skin is tightened, the effect may appear as if a patient is standing in a wind tunnel &#8212; the skin is swept back smooth and tight, but overlying a hollowed and sunken framework.  The more pure anatomic approach is to rejuvenate the face by restoring inner volume where it has faded. 			<content:encoded><![CDATA[<p>Most breast augmentations are straightforward procedures with beautiful results, but on occasion, a bothersome process known as <a href="http://www.donaldsonplasticsurgery.com/blog/2010/04/capsular-contracture/">capsular contracture</a> develops.  This occurs when the body tries to wall off an implant as a foreign object (like a splinter) with an abnormal amount of scar tissue.  The scarring can be firm, and even painful when severe.</p>
<p>Experts are not convinced about what causes capsular contractures: some think it is bacteria or &#8220;biofilm,&#8221; while others believe it is trauma from surgery.  Regardless, the reported incidence is anywhere from two to thirty percent depending upon the surgeon.</p>
<p>Dr. Donaldson takes many precautions to prevent capsular contractures in his own practice.  First, he uses meticulous sterile technique and antibiotics to avoid bacteria and infection.  He insists on &#8220;no touch&#8221; handling of implants so they go directly into position without delay.  He operates with a soft touch &#8212; &#8220;gentle with tissue&#8221; &#8212; to minimize trauma.  These simple principles have helped Dr. Donaldson achieve an extremely low rate of capsular contracture.</p>
<p>Columbus, Ohio patients also visit Dr. Donaldson for second opinions regarding capsular contracture treatment.  For these individuals, he performs an operation including capsulotomy (scoring and release of capsules) or capsulectomy (removal of capsules), with removal of the old breast implants and placement of new breast implants.</p>

<p>Several weeks ago, the company Allergan launched a new and improved version of Juvederm that contains lidocaine &#8212; a numbing medication similar to novocaine.  Their studies have shown that 9 out of 10 patients are more comfortable with the lidocaine than without, and so far Dr. Donaldson&#8217;s patients agree.  Procedures that might have otherwise required a nerve block (direct numbing of a facial nerve) can now be done safely and comfortably with the new filler alone.</p>
<p>In the last several days a competitor, Medicis, announced the availability of their latest products: Restylane-L and Perlane-L.  These next-generation fillers also contain lidocaine for a more pleasant cosmetic experience.
